Creation in ChipEdit

I don't want to be stupid, but what is the fault in ChipEdit that creates 00Tile? The artile says there's a fault in ChipEdit that creates this tile, such as in TCCLP's "Glitched To Death", but doesn't explain what the fault is. Can anyone explain? Thanks!
